1011 Front Derailleur - Catalog 12, p. 14

I have two of these rod-shifting derailleurs, one is complete and the other is missing a frame-mounting screw (item: 406) and the snap-on front plate (item: 412/1). Unfortunately, I don't have boxes for either.

Does anyone know where I can obtain replacements for these missing items? Is it even still possible to obtain them or, perhaps, another 1011 derailleur from which to salvage the parts?
